Anna is a double board-certified Family and Emergency Nurse Practitioner with nearly a decade of nursing and healthcare experience. Her clinical background spans critical care, emergency medicine, rapid response, nursing leadership, and education, giving her experience caring for patients across a wide range of needs and stages of life.
Before becoming a nurse practitioner, Anna built her nursing foundation in the Intensive Care Unit (ICU), caring for critically ill patients and families during some of their most vulnerable moments. After transitioning into advanced practice, she expanded her clinical career into emergency medicine while continuing to serve in healthcare leadership and nursing education. She is currently completing her Doctor of Nursing Practice (DNP), with a continued focus on improving the quality and accessibility of healthcare.
Anna founded Sugar Creek Health with a simple goal: to bring healthcare back to what it should be—personal, accessible, and built around the patient. She believes great primary care isn’t just about treating illness; it’s about building relationships, being available when patients need care, and helping people stay well before something is wrong.

Meet Jordann
Jordann Dean is a board-certified Nurse Practitioner with 10 years of nursing experience and 2 years of experience as a Nurse Practitioner. She brings a diverse and comprehensive clinical background to Sugar Creek Health.
Her nursing career includes caring for adult and pediatric patients in intensive care settings, providing specialized ECMO care, and practicing emergency medicine. These experiences have strengthened her clinical judgment and reinforced her commitment to providing compassionate, timely, and individualized care.
As a Nurse Practitioner, Jordann has continued her work in emergency medicine, with a particular focus on caring for underserved and medically undereducated populations. She understands that accessing healthcare isn’t always as simple as making an appointment. Transportation, distance, work schedules, limited resources, and lack of insurance can all prevent people from receiving the care they need. At Sugar Creek Health, Jordann is passionate about removing those barriers.
She believes telehealth can be a powerful tool for bringing healthcare directly to patients, especially those who may not have the ability or resources to travel to a traditional medical office. She is also committed to providing accessible care without making insurance a requirement, allowing more individuals and families the opportunity to seek medical attention, guidance, and education when they need it. Her philosophy is simple: healthcare should meet patients where they are—not the other way around.
Through Sugar Creek Health, Jordann’s goal is to create a healthcare experience that is accessible, compassionate, convenient, and centered around the individual patient.
